Please download all appropriate MSS & PADI course paperwork (see below) and bring all the approprite forms to the first day of your PADI course. (Click on each item to download it). If you don't have Adobe Reader on your computer, click here to obtain a free download).
IMPORTANT:
The PADI medical statement is NOT required to be signed by your doctor UNLESS you answer ‘YES’ to any of the questions. If you have answered ‘YES’ to ANY question (regardless of how long ago the condition relates to), you must be examined by your doctor before participating in diver training.
